6. Study plan ( more than 800 words ) (1200-1500 Words better)
7. Original medical reports scan copy(Hand written by doctor&stamp on it)
8. Police clearance certificate
9.Job experience (if have)
10.CV.
12. Extra curriculum activities (if have)
13. Others supporting documents ( if available)
